For a monthly fee, members will receive the primary care services described below.
Payments are made monthly via automatic transfer from your checking account, health savings account, or credit card.
There are no deductibles, co-pays, or co-insurance because it isn’t insurance; it’s a membership.
Most of the primary care services we provide in the clinic are covered, including:
• Office visits, counseling, and treatment recommendations per your condition
• Lab & Imaging evaluations
• Referral &Prescription management
• Minor surgery: cleaning of lacerations, sutures, removal of earwax, ingrown toenails, warts, simple cysts, skin tags, and moles
• General physical exams: screening exams, wellness exams, well child checks, DOT-CDL exams, sports physicals, scouting physicals
• Injectiontherapy**: ozone, trigger point injections, and segmental therapy
• IVtherapy**: basic nutrients, iron, chelation
• IM injections**: vaccinations, B12, MIC
**Member is responsible for the cost of the nutrients or vaccinations injected.
Blood draws are included, but members are responsible for their lab fees.
Memberships are limited in number.
Members can cancel the membership after twelve months with 30 days’ written notice.
Canby Clinic can terminate the membership for non-payment or any other reason with 30 days’ written notice. If we are unable to provide the services promised, Canby Clinic will issue a refund to members for fees paid on a prorated basis.
The disclosures required by Section 2(4), Chapter 499, Oregon Laws 2011 (a) that the practice is not insurance; (b) That the practice provides only the limited scope of primary care services specified in the retainer medical agreement; (c) That a patient must pay for all services not specified in the retainer medical agreement; and (d) “The Department of Consumer and Business Services issued a certification to this practice.
