Governing Law

Dispute Resolution
Any dispute arising out of or in connection with these Terms will be resolved by binding arbitration in accordance with the rules of the American Arbitration Association. The arbitration will be held in Portland, Oregon, and the decision of the arbitrator will be final and binding.

These Terms constitute the entire agreement between you and Canby Clinic with respect to the use of the Site. These Terms supersede all prior or contemporaneous communications, representations, or agreements, whether oral or written.
Severability
If any provision of these Terms is held to be invalid or unenforceable, such provision will be struck from these Terms and the remaining provisions will remain in full force and effect.
Waiver
No waiver of any provision of these Terms will be effective unless in writing and signed by Canby Clinic.
Assignment
You may not assign these Terms without the prior written consent of Canby Clinic. Canby Clinic may assign these Terms at any time without notice to you.
Notices
All notices and other communications under these Terms will be in writing and will be deemed to have been duly given when delivered in person, upon the first business day following deposit in the United States mail, postage prepaid, certified or registered, return receipt requested, addressed to Canby Clinic.
